I had somedesign and a person actually purchased it, however, after their purchase a day or two, I was notified that my designs got rejected due to “Design quality”. Will the buyer get their shirt? or will they get a refund? Thank you for reading

It depends. If it was rejected due to copyright for example, it might not get printed but in other cases it might. An order number will help here!
